Clinics & ServicesIntroductionYour Doctor will try and find the best way of making you better. This may not involve giving you medicine or tablets, as there are other ways to treat you, so please do not expect a prescription on every visit. You may require advice on how to prevent illnesses in the future. The surgery provides minor surgery, but if you need specialist advice or treatment, you will be referred to the hospital. Patients with minor injuries should attend Casualty. Practice Clinics
Practice ServicesClinicsAntenatal Clinics: If you think that you are pregnant you need to see your Doctor as soon as possible. Once your pregnancy is confirmed arrangements will be made for the Midwife to see you, often in your own home, and she in turn will arrange any necessary blood tests and scans and advise you about the frequency with which she and your Doctor need to see you. Antenatal Clinics are held at the Surgery on Wednesday afternoons and appointments should be pre-booked via Reception. It is very important that after your baby is born you see your Doctor for your postnatal check. Ideally the check should be carried out 6 to 8 weeks after the birth and no later than the twelfth week. Asthma Clinics: Our Practice Nurses hold qualifications in asthma care. In addition to monitoring your health, the aim of the Clinics are to ensure that you have a full understanding of your condition, that you have a good inhaler technique and that you are able to recognise at an early stage the warning signs of when you need help. Cervical Smear ClinicsThe Doctors encourage all women aged between 25 to 49 to have a cervical smear taken every 3 years, and women aged 50 to 64 every 5 years, or more frequently if indicated. A computerised call and recall system is operated and you will be notified when your next smear is due. When you receive your notification please book in with the Doctor or Practice Nurse of your choice and in making the appointment mention to the Receptionist that it is for a smear. If you have not had a smear for more than the number of years indicated above, or if you require more information, please speak to our Admin Office. ServicesBlood Pressure MonitorThis service is for patients who are required to monitor their blood pressure regularly and who send us their results on a three monthly basis. It allows the results to be sent in using an online form via this web site. Access the Blood Pressure Monitor. Change of Address / Telephone NumberIf you change name, address or telephone number, please complete our online form, or let our receptionists know by telephoning or writing to us. If you move outside of the practice area it may be necessary for you to find a doctor in your new area. South Staffordshire PCT will help you if you experience any difficulty. The instructions regarding moving to another area are printed on your medical card. Please read it and keep it in a safe place. Community Psychiatric NurseThe Community Psychiatric Nurse (CPN) attached to our Surgery and her work involves the treatment and counselling of patients with mental health and emotional problems. Depending on circumstances, she sees patients at the Surgery or in their own home. Referral to the CPN is strictly via the Doctors. Diet & NutritionThe Practice Nurses are able to provide advice on how to achieve weight loss and to those requiring low fat or other specialised diets. A wealth of information is also available about healthy eating. We have the facility to refer to a dietician. Appointments can be made after referral by one of the Doctors. Please also refer to the Diet & Nutrition resource links in our 'Your Health' section. Direct Access Day Case Service The service is only available for a number of specific operations and investigations and your Doctor will tell you if it is appropriate for you. District Nurses The District Nurses can be contacted by telephoning 01785 660575. Cover is provided 365 days a year and at bank holidays and weekends you can leave a message. The answer phone is regularly checked and messages responded to. Minor Surgery
Your Doctor will advise you whether we can treat you at the Surgery. We also hold cryosurgery clinics at 6 weekly intervals for the treatment of warts. If you would like a wart/warts removed please have a word with your Doctor who will, if appropriate, arrange an appointment at the next clinic. Exercise on Prescription
If you would like to be considered for the scheme please have a word with your Doctor or any of the Practice Nurses or Paul Bishop the Practice Manager. Family Planning
Emergency contraceptive (the morning after pill) is prescribed by all of the Doctors. To be effective you need to seek advice within 72 hours. You can also purchase the emergency contraceptive pill from any Chemist if you are aged 16 or over. Health visitors gives advice on health care to expectant mothers, babies and teenagers. A weekly well baby and childhood surveillance clinic is held at the Surgery on Thursday afternoons and a Doctor attends on alternate weeks to carry out 6 week baby checks. Mother and Toddler sessions are also held at the Surgery on Monday afternoons. Physiotherapy Referral to our Physiotherapist is strictly via the Doctor. Private ServicesWe charge private fees for services not covered by the NHS. For example :-

If you do not have your medical card, please fill in our permanent patient registration form or new-born baby registration form, both available from reception, or you may download a copy below:. Social Services Temporary ResidentsThe Doctors are happy to see patients who are temporarily resident in our Practice area. To make an appointment please contact Reception clearly stating that you are a temporary resident. To register you will need to complete our Temporary Resident Registration form which is available from reception, or can be downloaded below: Travel Advice Some travel vaccinations are not available on the NHS and in these cases we make a charge. Please ask the Practice Nurses or enquire at Reception for more details. The Practice Nurses also have a range of free literature to help you get the best out of your holiday. We recommend that you take a look at the 'Fit for Travel' website provided by the NHS (Scotland). You will also find links to travel related resources in the 'Your Health' section of this web site. Hazeldene House Surgery is an approved yellow fever vaccination centre and can provide this service to all patients irrespective of whether they are registered with us. Finally please plan ahead - don't leave your vaccinations to the last minute. Ideally you should attend the Travel Clinic about 8 weeks before your holiday. Well Women / Man Checks |
