Why is it Important to Cancel an Appointment ? By telling us in good time that you cannot keep an appointment, we can offer your appointment to someone else. This helps us to reduce waiting times and means everyone can be seen sooner.

Change of Address or Telephone Number:

If you change name, address or telephone number, please complete our Change of Address form, or let our receptionists know by telephoning or writing to us.

Are Your Medical Record Up-to-Date ?

Our Patient Health Questionnaire enables us to ensure your medical records are up to date. It is also a good way to tell us if you would like help with Giving Up Smoking.

Blood Pressure Monitor:

This service is for patients who are required to monitor their blood pressure regularly and who send us their results on a three monthly basis.
