The surgery will be closed over the lunchtime period 1pm to 2pm daily as of today Wednesday 18th March 2020.
Patients will be able to call the surgery during this time for urgent medical attention, any other calls we ask that you wait until after 2pm.
Answer a few short questions and we will get back to you within 2 working days. Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during our opening hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday to Friday between the hours of 6pm to 9am. Please contact 111 during out of hours or 999 if this is an emergency.

Hazeldene House Surgery
There is a bus stop on Main Road opposite the Hazeldene House Surgery building. It is a very short walk from the bus stop to us.
We have a large patient car park. The car park is only for the use of patients whilst attending the surgery, please move your car as soon as you leave the surgery.
All patient facilities at our main Surgery, apart from Treatment Room C, are at ground floor level and designed for ease of access. A fully equipped toilet for the disabled is located next to the main entrance. For patients who are hard of hearing we have an induction ‘Loop’ system. There is also a wheelchair kept on the premises for patients use within the building when necessary.
If you require any assistance whilst visiting the Surgery please inform the Receptionist.
Both Village Halls are also accessible for wheelchairs.
